What to expect from, and do before, your HGV medical with HGV Medical Derby

DVLA D4 Form

Get a copy of the DVLA D4 medical form. You can download a copy of the DVLA form using the button below. We have copies available for you, and this is included in the price of the medical. Please fill out the basic name & address information of the first page of the D4 form & write your name and date of birth at the bottom of each page of your printed D4 form before attending the medical.

Other Information

For vision requirements we have summarised what is required below. Please read and follow the instructions given below.

Collate and bring copies of any hospital letters that are relevant to the D4 form and your role as a bus/lorry driver etc.

Bring with you your full list of current medications. You can bring the boxes, a repeat prescription form, or a hand written, by yourself, list of medications

IMPORTANT!: Bring proof of identification, such as a current, valid driving licence.

Please bring the address and telephone number of the General Practitioner (GP) surgery at which you are registered. An individual named GP is not required, just the overall surgery details. You can fill this out on the D4 form before attending the medical if you have the time.

Vision Requirements

Eyesight requirements for you D4 medical with HGV Medical Derby

You don't wear corrective lenses

In line with the “Vision Assessment” section of the DVLA D4 form (click there to see the actual DVLA page):

If you DO NOT wear corrective lenses (glasses, contact lenses or both) for driving, your vision will be assessed during the medical by Dr Wilson, in line with D4 requirements.

For any of these sections, if you meet the D4 requirements, Dr Wilson will complete the “Vision Assessment” section of the D4 form. Should you FAIL to meet the requirements, you will need to consult an optician afterwards who will then need to complete the “Vision Assessment” section of the form – you will not need to return to Dr Wilson.

You wear contact lenses

If you drive while wearing CONTACT LENSES ONLY, please bring your current driving contact lenses, so your vision can be tested both with the contact lenses in and out – providing the contact lenses are “well tolerated”, that is all that is required; no prescription. If you might struggle to put your contact lenses back in afterwards, as you have no glasses to wear, please ensure you have someone to drive you to and from the medical.

If you drive while wearing CONTACT LENSES and/or GLASSES, you will be considered as a wearer of both and as such will be required to bring all driving related items (glasses, contact lenses and any related prescriptions) to the medical.

If you DO wear corrective lenses (glasses, contact lenses or both) for driving:

If you wear GLASSES ONLY to drive, Dr Wilson will require you to bring your current driving glasses, and the optician’s prescription for them, to the medical. Your vision will then be assessed in line with the D4 form for glasses wearers.

if you meet the D4 requirements, Dr Wilson will complete the “Vision Assessment” section of the D4 form. Should you FAIL to meet the requirements, you will need to consult an optician afterwards who will then need to complete the “Vision Assessment” section of the form – you will not need to return to Dr Wilson.
